A safe hospital environment is the most pressing and most challenging issue for healthcare workers. Since only it “guarantees” for doctors and nurses the reduction or absence of occupational diseases, i.e. maintaining health. That is why the student research circle "Search" within the framework of the cyclic methodological commission chose the direction of scientific research - the safety of a nurse in the workplace.

Nurses are exposed to many harmful factors: chemical, biological, physical, psychological. How to protect yourself from the influence of these factors, how to preserve your own health, how to minimize the risk of infection - these are the areas of research. For several years now, at the meetings of the circle, we have been expanding our knowledge on the prevention of occupational morbidity, creating electronic, visual aids, abstracting professional literature, holding conferences, and manufacturing aids. In order to study the material in more depth, we, together with our leaders, hold scientific and practical conferences. Members of our circle and practicing nurses of health care facilities take an active part in their preparation and implementation.

A red thread in the messages heard at conferences is the idea of ​​the problem of protecting the health of medical personnel.

In the process of research work, students conducted a survey among nurses of medical institutions in Belgorod in order to find out the frequency of injuries with non-disinfected instruments, the level of knowledge on the prevention of occupational infection, the use of personal protective equipment (PPE) when performing manipulations.

The results of the study of the responses showed that occupational injuries associated with the risk of infection are a fairly common phenomenon, especially where there is a high percentage of worn out medical equipment, a lack of personal protective equipment, or an underestimation by the medical staff of the importance of the nosocomial infection problem.

Thus, nurses and midwives constantly expose themselves to the risk of occupational infection. But this risk can be greatly reduced by being careful professional activity.

So far, labor medical workers in Russia it is little mechanized. “Back pain” among nurses today is of a massive nature, and their state of health is becoming increasingly biological and social significance determines the quality of their work and quality of life.

Meanwhile, the problem of overload and injury of medical workers can be solved by training personnel in health-saving technology - medical ergonomics. In 2008, the subject "Ergonomics in Nursing" in the college was introduced into the professional training of medical personnel.

The objective of this course is to develop practical skills for safe work in caring for patients, training in modern technologies for moving weights, as well as methods for relieving muscle tension in forced working positions, methods for rest and restoring health in the process professional activity.

In 2009, teachers together with students held a master class "Ergonomics when moving patients."

The purpose of the master class:

To draw attention to the importance of the problem of maintaining the health of paramedical workers;

Consider occupational risk factors for medical staff;

Demonstrate ergonomic technologies used in the professional activities of medical workers;

Familiarize yourself with modern ergonomic equipment used when moving a patient.

Based on the results of a survey conducted in the health care facilities of Belgorod and the region among nursing professionals, the following results were obtained:

74% of nurses experience constant back pain by the end of the working day, regardless of their age and length of service;

17% - periodic back pain by the end of the working day with work experience up to 5 years;

9% - never bother.

To the question of what causes spinal injuries

54% of nurses noted the absence or lack of ergonomic equipment;

29% - attitude towards their health;

17% - lack of knowledge of ergonomics.

Use assistive devices if they are available - constantly - 42%, sometimes - 58% of the medical staff.

Maintain the correct position of the body in the workplace 36% of nurses; 40% try to comply; 24% do not attach importance to the correct position of the body.

Thus, 91% of the interviewed nurses had problems with the musculoskeletal system.

According to the results of the study, it can be concluded that nursing professionals should be trained in the principles of medical ergonomics, reduce the risk of injury, enjoy work, and prolong active professional longevity.

From October 28 to October 30, 2009, the VIII Interregional Specialized Exhibition “Medicine. Pharmacy”, “Dentistry”, “Beauty and Health”. The organizers of the exhibition held a number of scientific and practical conferences, one of which was attended by teachers and students of our circle. We held a master class on the topic: "Ergonomic technologies as a factor in ensuring the effective work of nurses." The chief and senior nurses of the medical facility, who were present at the conference, followed, literally, every movement. We demonstrated ergonomic technologies used in the professional activities of medical workers, aids for moving and lifting the patient. Techniques and techniques have been proposed that allow you to move the patient with the least risk to health.

It is gratifying that the conference participants, practicing nurses, noted the relevance of our presentation, the need to introduce ergonomic technologies in practical healthcare.

The research activity of our circle has a practical orientation, close connection with the medical institutions of the city and the region. The introduction of health-saving, ergonomic technologies will allow nurses to maintain their active professional longevity.

A safe environment is special conditions organization of the work of medical institutions, guaranteeing the absence of the possibility of infectious infections and occupational diseases. Russia has one of the highest rates of morbidity among nursing professionals in the world.

A significant share in the structure of morbidity of medical workers is traumatism in the performance of professional duties. Nurses have to move patients, move heavy equipment, carry heavy loads, and stay in a forced position for a long time.

Until now, the work of medical workers is little mechanized, and this fact determines the work of personnel with the use of great physical effort, often in extreme conditions. Long-term influence of adverse factors leads not only to trauma, but also to diseases of the musculoskeletal system: spine, lower and upper extremities. Back pain is now an "epidemic" in healthcare. The state of health of medical workers is becoming increasingly socially significant, since it determines not only the quality of their work, but also the quality of life.

Today, the problem of overload and injury of medical workers is solved with the help of health-saving technology - medical ergonomics, its introduction into the educational professional training of medical personnel, the practical activities of nursing professionals in medical institutions.

In 2001, at the Perm Basic Medical College (PBMC), medical ergonomics was included in the curriculum for training in the specialty "Nursing", basic and advanced levels of education as an elective in the amount of 36 hours of study. The teacher of the highest category N.I. Shchelkunova prepared and approved the author's program for teaching students.

The objective of the course is to develop practical skills for safe work in caring for patients, the study of modern lifting technologies for moving weights, mastering the methods of unloading the body in forced professional positions, methods of rest and restoring health in professional activities .

The program of the elective "Medical Ergonomics" consists of 3 parts: the theoretical part (8 academic hours), the ergonomic workshop (26 hours) and the test in practical skills (2 hours).

The main goal of the theoretical classes is to create a theoretical base for mastering the ergonomic technologies of the work of nurses, understanding the essence of health-saving technologies, and fostering a healthy lifestyle. The main sections of theoretical training are:

    biomechanics;

    physiology of labor and physiology of the environment;

    professional psychology;

    theoretical foundations of ergonomic technologies for the work of nurses in medical institutions.

The aim of the Ergonomic Workshop is to directly teach graduates and nursing professionals in rational patient care techniques.

The ergonomic approach to mastering practical skills is based on 6 basic principles of safe work organization:

1) assessment of the task assigned to medical workers;

2) assessment of the environment and their capabilities;

3) determination of the degree of risk;

4) invitation to cooperation of the patient;

5) assistance from other members of the medical team or relatives;

6) the use of various medical ergonomic devices to facilitate the movement of the patient.

Ergonomic workshop includes:

    self-assessment of motor activity of students and anthropometry;

    training in methods of controlling the independent movement of patients;

    moving a patient by a nurse alone;

    moving a patient by two or more nurses in multi-medical teams;

    moving the patient with the help of equipment;

    lifting technologies for moving weights.

The teaching of medical ergonomics is hampered by the lack of textbooks on ergonomics in Russia that could be used to educate students of medical schools and improve the professional skills of practicing nurses. College teachers N.I. Shchelkunova and L.M. Zhelyaskova developed educational and methodological complexes for all practical exercises and algorithms for mastering ergonomic skills in the main sections of the workshop.

A modern approach to the organization of practical classes required the creation of a medical ergonomics office in the preclinical complex of the college. The office area (56 m2) allows you to simulate a patient's hospital room. The office is equipped with a functional and standard hospital bed, medical gurneys, stretchers, wheelchairs, couches, etc. The office has a soft floor covering for mastering practical ergonomics skills on the floor.

In addition, the office has special ergonomic equipment:

    ergonomic mats for practicing the skills of helping patients with a fall, moving patients, as well as for teaching skills and control over the independent movement of patients;

    stops for independent movement of patients in bed and with the help of one nurse;

    rope ladders for self-raising patients in bed;

    sliding sheets (maxi-slides) for moving a seriously ill patient to the head of the bed, moving from bed to wheelchair and from wheelchair to bed;

    rotating discs (flexi-discs) for turning patients while sitting and standing;

    rollers for moving seriously ill patients, turning along the axis while sitting in bed;

    restraint belts for fixing patients during lifting and escort;

    elastic plates for all types of patient movement; sliding boards for moving the patient from bed to chair and back;

    hammocks for moving patients to other rooms and wards;

    knee pads for performing manipulations in the “standing on one knee” position;

    cushions and pillows for the physiological position of the patient in bed.

Since in Russia there is still no production of ergonomic devices, many of them are made by the hands of our enthusiasts - college teachers. The administration of PBMK applied to the industrial enterprises of Perm with an order for the manufacture of ergonomic equipment, but was refused due to the high cost of manufacturing a small batch of equipment. Private business also did not respond because of the low profits of the manufacturer.

With the project “Medical Ergonomics - Innovative Nursing Technology”, PBMK participated in the competition for a grant from the Administration of the Perm Region, but failed to become a winner. There were no sponsors for such a necessary cause in the region either.

Ergonomic devices for the needs of the college were found among industrial gymnastic apparatus. Gymnastic mats produced by the polyurethane products plant in Izhevsk were adapted for ergonomic mats. To create flexi discs that meet all the requirements of ergonomics, it was enough to stick linoleum on the bottom surface of the gymnastic discs, which made it possible to give them the property of sliding along the floor. Ordinary gymnastic rope ladders and plates are also used.

Maxi-slides, rollers, holding belts, hammocks were sewn by teachers themselves from sliding raincoat and dense cotton fabric. Today, according to patterns, this ergonomic equipment can be sewn in any sewing studio. Rollers, stops, knee pads and foam cushions were made by graduate students. The college purchased functional beds, walkers, couches, wheelchairs.

In addition to the technologies of moving patients, a section of the workshop is the development by graduates of lifting technologies for lifting and moving weights with the help of equipment and mechanical devices. Graduates learn how to work with a functional bed, lifting wheelchairs with pedal hydraulic mechanisms, wheelchairs for moving patients.

In order to facilitate the work of nurses, training is being provided to work with lifting lifts for transporting medicines from a pharmacy, material from the CSO, moving equipment, etc., which is successfully used in the Perm Regional Oncological Dispensary. During practice in ergonomics at health facilities, graduates learn how to work in the correct professional postures, how to operate mobile manipulation tables, mobile treatment stands, how to work with infusion pumps that provide freedom of movement for patients during intravenous drip infusions, etc.

Over the past 3 years, more than 760 college graduates have received ergonomic training. Currently, medical ergonomics is included in the professional training curricula of all graduates of the Perm Basic Medical College at the basic and advanced levels of training in the specialty "Nursing".

More than 26 nursing teachers and heads of practice of medical colleges and schools of the Perm Territory were trained in medical ergonomics in the college as part of the advanced training of teachers of the discipline “Fundamentals of Nursing” in medical ergonomics. The college is ready to accept teachers from medical schools and colleges in Russia at the Faculty of Medical Ergonomics.

Guided by the position that the quality of work of nursing professionals in practical health care cannot be higher than the quality of their professional training, by introducing an elective in medical ergonomics into the training of graduates of the basic level, the college was the initiator of the introduction of ergonomics into practical health care, organizing training in ergonomic technologies for senior medical nurses of health facilities and practicing nurses. Under the agreement on joint activities College with the Perm Regional Oncological Dispensary, a cycle of training in ergonomic technologies for 12 senior nurses was organized and conducted free of charge on the basis of the dispensary, since the oncological dispensary provides care to the most difficult patients. It is in the oncological dispensary that the main nurse M.P. Shelkova was the first to start active work on the introduction of ergonomic and lifting technologies into the practice of nursing professionals.

During training at an advanced level in the specialty "Organization of Nursing" on the basis of contractual relations, 15 senior nurses of the Perm Regional Children's Clinical Hospital underwent ergonomic training. Together with the International Public Organization “Association of Nursing and Pharmaceutical Workers”, a scientific and practical conference on ergonomics was held for 180 nursing professionals from the Perm Region.

Today, practical healthcare is interested in introducing innovative nursing technologies into the daily practical activities of healthcare facilities. Don't wait for someone to make and provide nurses with ergonomic equipment. The problem can be successfully solved by the regional Association of Nurses, whose task is to protect the professional interests of nursing professionals, and the Association can take on many of the costs of ensuring safe working conditions in healthcare facilities.

Initiative and enthusiasm today allow nursing specialists trained in medical ergonomics, especially in such “load” professions as an operating nurse, an anesthetist nurse, a procedural nurse, a nursing assistant at a healthcare facility, to be in the correct professional postures, use ergonomic postures of rest and unloading of the spine, to work in multi-medical teams when moving patients, to use lifting equipment in everyday professional activities, which allows maintaining physical health and ensuring professional longevity of medical workers.
