How to fill out the T-11 form?

It is recommended to use the unified T-11 form to express gratitude to one employee and T-11a for a group of employees. To apply, you must have an application or a submission for promotion. The head of the organization makes a mark “for execution” on it, after which the personnel department prepares an appropriate order with subsequent approval by the director of the company.

If an additional premium is paid, then its amount is entered in the field below. If the monetary component is not provided for the employee, then the field indicating the amount is not filled.

The form and procedure for maintaining work books are approved in two regulatory legal acts:

As follows from subparagraph "c" of paragraph 24 of the Rules, any incentives provided for labor law or a collective agreement, as well as local acts of the employer, are entered in the work book.

Section 4 of the Instruction contains a detailed list of actions when reflecting the relevant information in the work book. He is like this:

  • in the third column of the "Information on the award" of the work book, the name of the employer is entered (full and abbreviated - if any);
  • in the first column, the cumulative total is the record number;
  • in the second column, the date of the promotion of the award is entered;
  • the third indicates who encouraged the employee, for what, as well as the type of encouragement. For example: “Gratitude for the great contribution to the development and promotion of the new PR project “Advertising Technologies Today” was announced;
  • the fourth column reflects the details of the relevant incentive document (for example, an order in accordance with which gratitude was announced and an entry was made in the work book).

As a rule, gratitude is issued by the employer at the request of the immediate supervisor of the employee.

For example, a structural unit of the organization managed to create and promote an important commercial project. What does gratitude give in a work book?

The legislation does not provide for any legal consequences for employees whose work books contain a record of gratitude. At the same time, the presence of incentives indicates the conscientious performance by the employee of his duties, which can be taken into account, for example, when imposing a disciplinary sanction on him for misconduct. In addition, the presence of such incentives for an employee can be taken into account by the employer as one of the incentives for career growth.

What entries does the work book provide in terms of encouraging employees?

Incentives in the organization have different kind. These include:

The payment of bonuses to employees for conscientious work is carried out on the basis of the order of the head. Here the question arises: if all employees of the organization are paid, for example, quarterly or annual bonus based on the results of the work provided for by the regulation on remuneration, is it necessary to familiarize all employees with the order and how? Gifts for employees

Giving gifts is also one of the effective incentives for employees. We are talking not only about valuable gifts given personally for labor merits, but also about gifts dedicated to the holidays on March 8, February 23, anniversaries, retirements, etc. When using such an encouragement, one should remember: due to paragraph 28 of Art. 217 Tax Code of the Russian Federation if the value of a gift received once during the year is more than 4,000 rubles, from the part that exceeds this amount, the employee will have to pay personal income tax ( letters of the Ministry of Finance of the Russian Federation dated November 22, 2012 No. 03-04-06 / 6-329 and dated 13.03.2009 No. 03‑04‑06‑01/59). The employer withholds this amount from the employee's salary ( paragraph 4 of Art. 226 Tax Code of the Russian Federation).

So that the cost of purchasing gifts for staff associated not with holidays, but with production results, high achievements in labor, and other similar payments were taken into account in income tax expenses, in employment contracts with employees there should be a condition that they are covered by the incentive system in force in the organization. This explanation is given by the Ministry of Finance in Letter No. 22.04.201003‑03‑06/2/79 .

With regard to gifts for holidays, the Ministry of Finance has repeatedly explained that such expenses cannot be taken into account when calculating income tax, since they are not related to the production results of employees (see, for example, Letter No. 21.02.201103‑03‑06‑01/59 ).
