Financing

In 2006, for the implementation of the project from the federal budget and state off-budget funds 78.98 billion rubles were allocated. Besides, additional funds were allocated by the subjects of the Russian Federation and municipalities. In 2007 it is planned to allocate 131.3 billion rubles.

For 2007-2009, expenses related to the implementation of the project are planned in the amount of 346.3 billion rubles.

Immediately after the start of the national project, it turned out that the financing of several important items of expenditure was not taken into account during its development. For example, incentive payments received by medical staff, contrary to Art. 139 Labor Code not included in average earnings and, therefore, these payments did not affect the amount of "holiday" and "sick leave". In this regard, the Ministry of Health was sent great amount appeals both from regional heads of health care and from ordinary employees. The first to achieve justice on this issue was the therapist Anatoly Popov from the Vladimir region.

The issue of the so-called “northern allowance”, which is a significant part of the salary of the population of the northern regions, was much more difficult to resolve. This allowance should apply to all types of earnings, regardless of its source, but none of the budgets provided funds for this purpose. In a number of regions, after medical workers apply to the courts, the debt on the “northern allowances” for incentive payments is reimbursed only to those who apply to the court. Health authorities continue to challenge, with varying success, court decisions that ordered employers to pay back the northern surcharge. Funding problems continued in 2008 and 2009, part of the costs were shifted to the shoulders of regional, city and even municipal authorities, which did not contribute to the achievement of the project's goals.

Implementation results

Life expectancy in Russia

At the beginning of July 2007, 5,834 doctors had completed training and retraining (2,939 in Therapy, 2,298 in Pediatrics and 597 in General Medical Practice). In the first half of 2007, more than 150,000 medical workers (more than 70,000 doctors and 79,000 nurses) received additional cash payments totaling more than 6.6 billion rubles.

At the beginning of July 2007, 3267 units of diagnostic equipment were delivered to the regions (512 sets of laboratory equipment, 71 ultrasound machines, 788 X-ray machines with 443 processors, 438 units of endoscopic equipment, 465 electrocardiographs and 550 fetal monitors). Criticism

Despite the good intentions of the curators of the national project, it was not possible to significantly change the situation in healthcare. Some experts call the national project "Health" a failure.

Against the backdrop of optimistic reports from the ministry on the success of the national project, deputies of the State Duma said that the situation with drug provision and the staffing of medical institutions leaves much to be desired.

It must be admitted that the situation here is far from favorable. It needs to be radically changed. I consider it necessary to pay special attention to the development of primary health care, primary health care, disease prevention, including vaccination and effective medical examination of the population. In the next two years, more than 10,000 municipal polyclinics, of which more than a third are in rural areas, as well as a significant number of district hospitals and feldsher stations, will need to be equipped with new diagnostic equipment. This is almost all primary health care. We have a little more of them, about 17.5 thousand, but the difference is a departmental network.

Early 2006 wage district physicians, pediatricians and general practitioners should increase by an average of 10 thousand rubles a month, and nurses - by at least 5 thousand rubles. At the same time, its specific size should directly depend on the volume and quality of medical care provided. A particular problem is the availability of high medical technologies in cardiac surgery, oncology, traumatology, and a number of other important areas, and, above all, their use in the treatment of children.

The number of citizens who will be provided with high-tech medical care at the expense of the federal budget should increase by 2008 by at least four times. To do this, it is necessary to raise the efficiency of both existing centers of high medical technologies and build new ones, primarily in the regions of the Russian Federation, including Siberia and Far East. I propose to allocate substantial funds from the federal budget to the solution of the tasks outlined above. At the same time, I immediately draw attention to the need for plans to be concise and completely specific. Modern construction technologies, equipment technologies of the medical unit make it possible to do this in as soon as possible. In two years we are able to solve this problem. Improving the quality of life of Russian citizens is a key issue of state policy. It would seem an indisputable declaration. That is how it is perceived now. Dangerous disintegration of state institutions, a systemic economic crisis, the costs of privatization combined with political speculation on the natural desire of people for democracy, serious miscalculations in the implementation of economic and social reforms - the last decade of the 20th century was a period of catastrophic demodernization of the country and social decline. Almost a third of the population was below the poverty line. Many months of delays in the payment of pensions, allowances, and wages have become a mass phenomenon. People were frightened by the default, the loss of their savings overnight. They no longer believed that the state would be able to fulfill even minimal social obligations.

This is what the government, which began working in 2000, faced. These were the conditions under which it was necessary to simultaneously solve the most acute everyday problems and work to lay down new - long-term - growth trends.

The path that the country has traveled over the past five years is very briefly but clearly described by excerpts from the annual messages of the President of Russia to the Federal Assembly. Let's remember what words Vladimir Putin used to address the nation in different years.

In 2015, within the framework of the priority national project "Health", 1438.6 million rubles were invested in the health care of the region, of which: 1243.7 million rubles - from the federal budget and the Fund social insurance; 194.9 million rubles - from the regional budget. The implementation of the activities of the priority national project "Health" was carried out within the framework of the state program of the Russian Federation "Health Development" for 2013-2020 in the following areas:

Development of primary health care and improvement of disease prevention

2,689,598 people were vaccinated within the framework of the National Calendar of Preventive Immunizations, 271,966 people were vaccinated according to epidemic indications, which made it possible to maintain epidemiological well-being in terms of infections controlled through vaccine prophylaxis, to avoid group and outbreak morbidity of the population of the Rostov region in 2015.

At the expense of the federal budget, vaccines were delivered to the region in the amount of 286,998.4 thousand rubles, which is 38.6 million rubles more than in 2014; 44,647.1 thousand rubles were provided for these purposes from the regional budget. , 34,647.1 thousand rubles were financed. The complex of organizational, therapeutic and prophylactic, sanitary and anti-epidemic measures implemented in the region makes it possible to timely identify those infected with immunodeficiency viruses, hepatitis B and C and provide the necessary medical care. In 2015, innovative methods of HIV prevention were applied using television and radio channels with a total coverage of more than 95% of the population of the Rostov region.

From the federal budget, test systems and antiretroviral drugs were purchased in the amount of 230,771.9 thousand rubles, and 10,690.5 thousand rubles were spent on prevention measures. For the purpose of combating HIV infections, 31,091.1 thousand rubles were allocated from the regional budget.

Activities aimed at detecting tuberculosis,treatment of patients with tuberculosis

Thanks to a systematic approach that includes measures to prevent the spread of tuberculosis, its timely diagnosis and treatment, it is possible to reduce the incidence and mortality from tuberculosis every year. In 2015, according to operational data, the incidence of tuberculosis was 39.6 cases per 100 thousand of the population, and the death rate from tuberculosis was 15.1 cases per 100 thousand of the population (2014 - 16.8).

The number of people examined for tuberculosis by fluorography in 2015 amounted to 1,823,062 people, which is 78,201 people more than in 2014.

To finance the re-equipment of medical institutions of this profile and the purchase of antibacterial anti-tuberculosis medicines(second row) in 2015, 206,167.0 thousand rubles were allocated from the federal budget, and 6,636.0 thousand rubles from the regional budget.

Increasing the availability and quality of specialized, including high-tech, medical care

Since 2014, the possibilities of financial support for the provision of high-tech medical care at the expense of compulsory medical insurance have been expanded. In 2015, 7,190 people received high-tech assistance as part of the financing of the Health national project (from all sources of funding - 16,304 people, in 2014 - 13,119 people). The availability of specialized medical care for children in the region has increased. 2,468 children were treated (2014 - 1,955 people).

The provision of high-tech medical care was carried out in 17 medical institutions of federal, regional, municipal subordination, private forms of ownership. Over the years, the main development in the region has been those areas that directly affect the medical component of demographic indicators. As a result of the implemented set of measures, it was possible to improve the annual target values ​​of infant mortality rates. Infant mortality decreased by 16.5% compared to 2014 and, according to current data, was 6.6 cases per 1,000 live births.

Many activities previously included in the Health national project were implemented in 2015 as part of the State Health Development Program until 2020. In the main significant areas, separate agreements were concluded to co-finance the expenditure obligations of the subject from the federal budget. Part of the activities was financed from the funds of compulsory medical insurance.

Formation healthy lifestyle life

In 55 territories of the region, more than 648 actions of the movement for a healthy lifestyle "Quiet Flows the Don - health in every home!" were held; 11 field events within the framework of the project "Your health is in your hands - Enlighten!"; 7 specialized events within the framework of the project "Let's keep health at work"; 55 events within the Don Donors, Dance for Life, Everything That Concerns You projects, Healthy Start and Youth Against Drugs volunteer campaigns.

Medical examination of citizens and children

The creation of a regional register of patients with acute cardiovascular pathology has significantly increased the number of cases of delayed surgical interventions aimed at secondary prevention of recurrent cardiovascular accidents.

As a result, according to operational data, a decrease in the indicator "Mortality from diseases of the circulatory system" was achieved - 617.3 cases, while the plan was 753.4 cases per 100 thousand of the population, which is significantly lower than the national average - 631.8.

Measures to improve the organizationcancer care to the population

Thanks to the well-established mechanism for the interaction of all services in providing medical care to victims of road traffic accidents, there is a tendency to reduce the number of deaths at various stages of medical care. Plans and tasks for 2016

In 2016, within the framework of the national project "Health", the provision of medical care to women during pregnancy and childbirth will continue. Financing in the amount of 488.1 million rubles is planned from the FSS funds. All the main activities of the PNP "Health" are included in the state program for the development of healthcare until 2020.

By 2006, Russian healthcare came with the following accumulated problems :

    underfunding of the industry;

    outdated material and technical base of medical institutions;

    low salaries of medical workers;

    structural imbalances;

    examination of newborn children for galactosemia, adrenogenital syndrome and cystic fibrosis;

3. Providing the population with high-tech medical care:

    The prestige of the work of primary health care workers will increase, as a result, young qualified specialists should come to the district service;

    Outpatient clinics will be equipped with the necessary diagnostic equipment, which means that waiting times will be reduced and the quality of diagnostic tests will be improved;

    New machines will be delivered to the regions Ambulance”, this will increase the efficiency of the ambulance service;

    Organization of a mass examination of newborns to identify hereditary diseases (phenylketonuria, congenital hypothyroidism, adrenogenital syndrome, galactosemia, cystic fibrosis) will prevent the disability of children;

    The organization of additional free immunization of the population will lead to a decrease in the incidence;

    Timely detection and prevention of diseases will become possible due to mass free medical examination of working citizens (aged 35-55 years) and citizens working in industries with harmful and dangerous production conditions;

    The detection of diseases will increase by 1.5 times and the average life expectancy of patients after the first diagnosis will increase by 3.5 years (up to 12 years), the number of days of temporary disability will decrease by 30%;

    It will be possible to reduce the incidence of hepatitis B by 3 times, rubella - up to 10 cases per 100 thousand of the population, the elimination of congenital rubella; the incidence of influenza stabilizes in the absence of the emergence of a pandemic virus; the frequency of HIV infection will decrease (per 1000 people per year);

    Thanks to the construction of new federal high technology centers, waiting times will be reduced and the availability of high-tech (expensive) medical care will increase, especially for residents of remote areas;

    Transparency of the queue for receiving high-tech (expensive) types of medical care will be ensured through the introduction of a system of "waiting lists";

    The quality of medical care provided to pregnant women, both in antenatal clinics and in maternity hospitals, will be improved through the introduction of a system of birth certificates.

In the future, during the successful implementation of the Health project, the following is expected:

* Reducing mortality and disability rates population by increasing the availability and quality of medical care;

* Need Satisfaction population of the Russian Federation in a free high-tech (expensive) medical care;

* Approximation national healthcare to European standards medical assistance to the population;

* Reduction of economic losses by strengthening and restoring the health of citizens, reducing the financial costs of paying disability pensions and temporary disability benefits;

* Strengthening the position of Russia in the domestic and international markets of medical services and medical technologies (economic and professional incentives for the implementation of domestic developments at the international level and attracting foreign patients to Russian clinics).
